Agreement For Placenta Print Services
Doula On The Move Birth Services
The following agreement outlines basic care practice and expectations in regards to your commissioned placenta print. Please contact me at email@example.com if you have any questions or concerns regarding the agreement or the process of creating your placenta print.
STORAGE AND CONTACT
If delivering at a Hospital, plan to bring a small cooler and two large ziploc bags to store your placenta safely. (A 12 can pop cooler is ample size)
If delivering at home, please ask your midwife to place the placenta in two large ziploc bags and refrigerate until pick up. Please do not freeze unless you have arranged that with me, as it will add significant time to the process and may compromise your print.
At the hospital, or at home, remind your caregiver that you are wanting to keep the placenta (and cord if possible)
Please add my cell number 778-893-6852 to your phone, so you aren’t searching on the day. I can be contacted at my office hours between 9 am and 9 pm to arrange pick up of the placenta at your location.
It is extremely important for you to inform me prior to placenta pick up if you intend to have your placenta encapsulated following the print process!
If you intend to have your placenta encapsulated following the print process, please provide me with the contact information for your chosen encapsulator. Failure to inform me of your intent to encapsulate may result in your placenta being unable to be processed into capsules due to ink/paint type and timing for foodsafe practices.
While Doula On The Move Birth Services will do everything reasonable to ensure the safe handling and integrity of your placenta while it is in my care, I will not be held liable for any mishandling prior to pickup or future processing or use of your placenta by other parties.
You are obligated to inform me if there are any active pathogens (GBS, Hepatitis, HIV etc) that I should be aware of for my safety while handling your placenta. This information will be kept confidential.
Doula On The Move Birth Services has the right to use and publish images and photographs taken of your placenta and the prints created to further promote its service, including portfolio, web site, display, advertisement, and editorial use. Use of your name or any other specific identifying information will not be done without your express permission.
Doula On The Move Birth Services retains the copyrights to all images created including, but not limited to photographs, video and original prints. The images and original prints provided are for any reasonable personal purposes, including but not limited to: printing, copying, emailing, and web publishing. You are permitted to print any of the images, for personal uses only. For any online sharing, Doula On The Move will be credited with the creation of the image / print. This release does not allow publication of your print (i.e. entering into contests), selling or altering your images (ie with filters etc) without the permission of Allison Runnalls and/or Doula On The Move Birth Services. Prints and digital images may not be scanned, copied, edited, or reproduced in any way.
PAYMENT AND CONFIRMATION
Confirmation of your commissioned print will be sent upon receipt of your retainer and a signed reply to this original agreement either in print or via email with “we have read and agree”.
Please allow up to 6 weeks after receipt of placenta by Doula On The Move Birth Services for delivery of your commissioned original print.
